Geting started

Getting started to having the best health care plan for your employees starts with a Census of your current employees.

We can either E-mail you an Excel format census form or deliver a printed form to your office. This form is a listing of the employees and dependents etc. We also would like to know what benefits you have in place now and what you would like to provide your employees. 

With the Data, we will obtain several optional plans to meet your criteria.

Group enrollment forms completed by the employer

Enrollment forms / packets will be distributed to the employees to be completed.

Effective dates can be either the 1st or 15th of the month. Either calendar or fiscal year may be used.

Multiple locations in or out of Florida may be used
